Sunshine Health has partnered with Blue Circle Health to work virtually with you to help our members manage their type 1 diabetes (T1D) and achieve their T1D health goals. The program is designed to complement your existing care, and offers a personalized plan that may include free clinical care, social work, education, nutrition and peer coaching services, based on a patient’s specific needs.
On average, goals are achieved in 3-6 months, and patients can re-enroll for free, if needed.
What to expect: If your type 1 diabetes patient participates in the Blue Circle Health program, you can expect monthly communications that may include clinical updates, case presentations and/or forums for discussion. Each program is personalized to patient needs and intended to complement your care team. Whether your patient has a quick care question or needs more in-depth nutrition or insurance support, Blue Circle Health is here to help. There is no cost to you or eligible participants.
Who is eligible?
- Diagnosis of type 1 diabetes
- Ages 18+
- English or Spanish speaking
- Ability to connect in virtual care
- Resides in Florida
- No current pregnancy
