Living with type 1 diabetes can be challenging. Blue Circle Health provides free, personalized care, education and support to adults living with T1D – at zero cost to your patient. We act as an extension of the care team to provide extra tools and support.

Eligibility Critera

To sign up for Blue Circle Health, people must meet the following requirements:

Be diagnosed with Type 1 Diabetes
Be 18 years or older
Live in Florida, Maine, or Vermont
Speak English and/or Spanish
Have access to the internet and a smart phone or computer to do video calls with the care team
Have a primary care provider or are willing to be connected to one
Are not currently pregnant
